Terms and conditions for Body Back®/Strides 360 Monthly Membership

FIT4MOM ASHBURN MONTHLY MEMBERSHIP AGREEMENT

This contract is made between the client and FIT4MOM Ashburn and is a month to month agreement, with a minimum 3 month commitment, and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited Body Back Workout® and Strides 360® classes.

AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Ashburn to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM Ashburn administrator.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Ashburn to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

CANCELLATION POLICY
To cancel this membership, the member must contact the FIT4MOM Ashburn administrator at least 30 days prior to their next payment due date to request a cancellation. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the FIT4MOM Ashburn franchise. If a membership is cancelled and the member wants to re-enroll at a later date, they will be charged half of the normal registration fee and be subject to future pricing.

HOLD POLICY
To put a membership on hold, the member must contact the FIT4MOM Ashburn administrator at least 15 days prior to their next payment due date to request a hold. Memberships may be held for a minimum of 1 month and a maximum of 4 months per hold period, per calendar year. Hold starts will be the 1st of the month. When a hold period is ended, the monthly auto payments will resume automatically on the return date. There will be a $20 processing fee charged to your credit card on file for the hold period and returned as a credit on the next bill when the hold ends.

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM Ashburn within the initial ten days. You are responsible for paying for the classes already attended.

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
